Telemedicine visit?

1) Please check in for your visit through your patient portal or e-mail link ahead of time to update your medications and history, etc.

2) If unable to do so, please call the clinic 5-10 minutes prior to your scheduled appointment to check in, then click on the link of the provider you are seeing below.

Need to pay your bill?  Please do so through the portal.  If you have trouble paying your bill, please download or call and request ahardship application.  If you download and print, you can return these requests for bill reduction or waiver to us via fax at 503-474-3601, mail, or bring in.
